Four rules for finding primary retinal break and 4 rules to detect
break in reoperation are presented, followed by closing break with:
Minimal segmental buckling – implying buckling limited to break
with sewed on sponge buckle(s) or a temporary sutureless buckle –
and without drainage – feasible by use of elastic buckle inducing
postoperative reattachment by spontaneous absorption of
subretinal fluid. Radial buckle provides optimal tamponade of break
which is treated intraoperatively by cryopexy or by laser after
reattachment. Indications, surgical technique of minimal segmental
buckling without drainage, complications, long-term results are discussed.
